4.2. How To Enter Risks/Issues Data

  1. Go to the risks and issues register.

  2. Login to KoBoToolbox using the credentials provided by PIU M&E focal person or MIS administrator

  3. Select the IP who is reporting (Leave blank if unavailable or this is a general entry)

  4. Select the tracking tool you wish to use (Risks/Issues or Lessons Learned)

  5. Fill in the details of the tracking tool chosen. Note that some fields are compulsory.

  6. Click on submit button to save the record.

  7. Validate the record so that it can appear on the dashboard.
